State Healthy People 2010 Tool Library
An online peer resource center to view and download the latest Healthy People 2010 resources from the field. This activity was supported through a cooperative agreement with the Office of Disease Prevention and Health Promotion.

PHF is no longer funded to collect new items for the Healthy People 2010 Tool Library. This archive project site contains tools and documents identified through 2004, many of which remain highly relevant to today's health improvement planning initiatives. Please report dead links to our webmaster.

Most links on this page are PDFs. To view or print pages in PDF format you must have Adobe Acrobat Reader 4.0 or above. You may download the most recent version of Acrobat from the Adobe web site.

Materials are organized by the seven action areas in the Public Health Foundation’s Healthy People 2010 Toolkit.

Identifying and Securing Resources

Funding to Support Wisconsin’s State and Local Public Health System Infrastructure (Wisconsin) 
Table lays out activities and short-, middle-, and long-term goals in order to meet the Healthiest Wisconsin 2010 objective of assuring equitable, adequate, and stable funding to support state and local public health system infrastructure—including planning and assessment efforts.  Steps include increasing the flexibility of how state and local resources are used; analyzing public health funding needs; and creating a committee to report to the state legislature on health outcomes.

Identifying and Engaging Community Partners

Engaging and Sustaining Selected Community Stakeholders in the Transformation of Wisconsin’s Public Health System (Wisconsin)
Supplemental report to the State Health Plan, Healthiest Wisconsin 2010: A Partnership Plan to Improve the Health of the Public. Describes the purpose, process, findings, and recommendations from five forums designed to gain insights on health disparities and to build public health relationships with African-American, Hispanic, Asian, American Indian, and gay, lesbian, bisexual, and transgender communities.  Offers broad recommendations to Wisconsin’s public health system partners to assure diverse and sustained partnerships for the future.  Part III describes how the Wisconsin Turning Point Initiative established these community stakeholder forums, as well as the successes and missteps in the process.

Stakeholder Interview Guide (Delaware)
Stakeholder interview guide developed by the Public Health Foundation (PHF) to learn how Healthy Delaware 2010 could engage, appeal to, and be useful to private and public sector audiences.  One-hour individual interviews and two-hour focus groups were conducted using the guide and analyzed by PHF.

Stakeholder Interview Prompt Sheet (Delaware)
Prompt sheets used in both individual interviews and focus groups with questions 1, 3, and 4 of the interview guide.  Useful to jog participants’ memories about issues that may be important but difficult to remember or think of without help.  Sheet #4 lists ways to assist with the state health plan and could be adapted for state use as a "pledge sheet" or summary of "things you can do."

Healthy Delaware 2010 Market Research Findings (Presentation) (Delaware)
Presentation of preliminary findings of Healthy Delaware 2010 "market research" activities: stakeholder interviews, focus groups, and marketing audits.  Used to inform the state’s development plan, publication plan, and marketing plan.  (Listed also under "Communicating Health Goals")

Roles for Key Actors (Iowa)
Describes the roles of the Healthy Iowans 2010 Consortium, work groups, chapter teams, review teams, editors, and resource persons.  Reviews how parties such as the Governor, Iowa Legislature, citizens, state agencies, and boards of health were involved in the Healthy Iowans 2010 planning process.

State Health Issues Opinion Survey: Healthy New Jersey 2010 (New Jersey)
Report summarizes findings of a telephone survey conducted in 1999 by The Center for Public Interest Polling, Rutgers, The State University of New Jersey.  The Telephone Survey Instrument may help other states to identify opinions about the most important health issues and roles of different entities in addressing health issues.

Promotional Brochure (Delaware)
Color brochure introduces the state’s Healthy People initiative to potential partners.  Describes the planning process and ways to get involved. Includes a response card.

Partnership Opportunities (Delaware)
Checklist of specific opportunities to become involved in the Healthy Delaware 2010 initiative, such as donation of resources and services, participation in committees, and policy advocacy.  Distributed to leaders invited to a small kick-off breakfast with the Governor.

"Healthy Delaware Heroes" Nomination Form (Delaware)
Form to solicit nominations of individuals, businesses, or organizations to feature in the Healthy Delaware 2010 plan.  In future, the state plans to give annual "Healthy Delaware Heroes" awards to recognize prevention efforts.

Draft of matrix Florida is using to cross 2010 objectives with the 10 Essential Public Health Services to develop a strategic plan.

Process Guidance (Maryland)
Packet of materials to guide the development of Maryland’s Health Improvement plan.  Contains answers to frequently asked questions, a list of Maryland priority issues, a sample format for each module (state and local), staff resources, and other guidance for state agency staff and local health departments.  Not available on this site are Attachment 9, a data guidance drawn in part from the Healthy People 2010 Toolkit, and Attachment 10, criteria for objectives development, drawn from the Healthy People 2010 Objectives: Draft for Public Comment.

Detailed agendas and facilitator guides for two phases of the state's work group process.  Phase 1—teams from five sectors (business, government, education, etc.) formed audience work groups to identify health improvement measures and strategies important to them.  Phase 2—representatives from each sector team formed "mixed groups" to agree on shared responsibilities and objectives.

Chapter Review Checklist (Iowa)
Checklist to help reviewers evaluate draft Healthy Iowans 2010 chapters.  Used by members of the 23 chapter review team to ensure chapters followed a consistent format.  May also be useful to states developing a list of standard information to be included with all priority areas and objectives.

Obtaining Baseline Measures, Setting Targets, and Measuring Progress

Online Analytical Statistical Information System (OASIS) (Georgia)
Interactive website contains vital statistics (births, deaths, infant deaths), Georgia Comprehensive Cancer Registry, and population data.  Users can select measures to either view in a table or download into a spreadsheet.  Another tool generates maps reflecting how each county is doing in terms of prenatal care, infant mortality rates, and other indicators.  Useful example of how to make health data accessible to the public.

Community Choices 2010 (Washington)
Report that is put out every three years by Community Choices 2010, a community-based non-profit organization whose mission is to improve health in Clark County, Washington.  This 2003 report card shows progress towards local health goals and comparisons to Healthy People 2010 objectives.

Healthy People 2010 Leading Health Indicators (California)
Report by the Center for Health Statistics, California Department of Health Services that uses the January 2004 Edition of the federal DATA2010 system to measure how California fares on 10 Leading Health Indicators and associated HP2010 objectives.

Annual Health Status Measures (Presentation) (Georgia)
Presentation uses graphs to compare Georgia statistics on health indicators with national statistics and with the state’s 2010 goals.

Healthy Michigan 2010: A Health Status Report (Michigan)
Report provides an overview of Michigan’s health status and a baseline for Michigan’s health improvement.

Target Setting Algorithm and Worksheet (Delaware)

Healthy Delaware 2000 Update (Presentation) (Delaware)
Describes a simple method to classify the status of state year 2000 health objectives as "significant improvement," "some improvement," "little/no improvement," "getting worse," or "cannot evaluate."  Presents the status of Healthy Delaware 2000 objectives organized by 27 health problems and level of opportunity to improve.

Tools of the Trade (Pennsylvania)
A series of brief technical assistance documents on commonly used methods to collect, measure, present, and analyze health statistics. Small Area Analysis (Pennsylvania)

The Michigan Surgeon General’s Prescription for a Healthier Michigan (Michigan)
Concise report that gives a snapshot of Michigan’s current health status and what the state needs to do to preserve and promote health.  Includes specific action items for schools, businesses, state and local health agencies, faith-based communities, and others.

Marketing Audit Script (Delaware)
Script for telephone surveys (marketing audits) conducted by The Nixon Group, a private marketing firm, to gather information needed to develop a marketing plan for Healthy Delaware 2010.  Used to learn perceptions of Healthy Delaware, how to effectively reach target audiences with Healthy Delaware 2010 campaigns, and opportunities to engage partners in marketing efforts.
