Developed by Public Health Foundation (PHF) Senior Quality Advisor Jack Moran and PHF Performance Improvement Expert Les Beitsch, the Change Management Questionnaire Checklist encourages change leaders to identify whether people are continually engaged, and determining if clear mid-course adjustments are being made, which supports the change initiative.
