Change Management Questionnaire Checklist

Overview

The Change Management Questionnaire Checklist supports efforts to make a substantive change to an entrenched culture. By answering a set of guiding questions, change leaders can build and sustain a conducive change environment. Developed by Public Health Foundation (PHF) Senior Quality Advisor Jack Moran and PHF Performance Improvement Expert Les Beitsch, the Change Management Questionnaire Checklist encourages change leaders to identify whether people are continually engaged, and determining if clear mid-course adjustments are being made, which supports the change initiative. The Change Management Questionnaire Checklist is one of many supplements to the 2012 Public Health Quality Improvement Encyclopedia, and accompanies the white paper, Are You Really Ready to Make a Change?
