The Public Health Foundation (PHF) and Catholic Health Association (CHA) hosted a webinar on May 29, 2018 focused on taking action in support of community health improvement. Featuring two communities participating in the Using The Community Guide for Community Health Improvement initiative, this webinar offered an overview of this pilot initiative and shared stories of action being taken in Baton Rouge, LA and Portsmouth, VA.
 
Through this initiative, Our Lady of the Lake Regional Medical Center in Baton Rouge, LA and Bon Secours Maryview Medical Center in Portsmouth, VA worked with public health and other community stakeholders, explored evidence-based strategies, and took specific actions to address HIV/AIDS and obesity and improve the health of their communities. Throughout the initiative, these hospitals drew on evidence-based resources, such as The Community Guide (The Guide to Community Preventive Services), and quality improvement methods and tools, including a population health driver diagram framework, to address priority health needs in their communities.
 
During this webinar, representatives from Our Lady of the Lake Regional Medical Center and Bon Secours Maryview Medical Center shared their experiences with the initiative, including successes and lessons learned. Methods and tools that supported the initiative, including The Community Guide and the population health driver diagram framework, were also featured.

Using The Community Guide for Community Health Improvement Initiative
The Using The Community Guide for Community Health Improvement pilot initiative is aimed at helping hospitals use evidence-based recommendations and findings found in The Community Guide (The Guide to Community Preventive Services) to take action and implement community health improvement activities in collaboration with health departments and other community stakeholders. A joint effort of the Centers for Disease Control and Prevention, Public Health Foundation, Association for Community Health Improvement/Health Research & Educational Trust, and Catholic Health Association, the Using The Community Guide for Community Health Improvement initiative is using the population health driver diagram framework, developed by PHF, to help hospitals and their community partners align and implement actions to address identified population health priorities. Four hospitals have been engaged in pilot testing this framework and have worked with community stakeholders to address health priorities ranging from behavioral health to obesity to HIV/AIDS.
