Through this initiative, Our Lady of the Lake Regional Medical Center

(The Guide to Community Preventive Services), and quality improvement methods and tools, including a population health driver diagram framework
, to address priority health needs in their communities.
During this webinar, representatives from Our Lady of the Lake Regional Medical Center and Bon Secours Maryview Medical Center shared their experiences with the initiative, including successes and lessons learned.
