A Model Population Health Initiative: Reducing Costs by Going Upstream
Upcoming Webinar on October 23, 2017 at 1:30pm EDT

This webinar will highlight the winner of the 2017 Future of Population Health Award, telling the story of Harbor Place, which provides temporary transitional housing for homeless patients discharged from the University of Vermont Medical Center. The hospital recognized the need to address homelessness -- an upstream cause of repeat hospitalizations for chronic diseases. The program has significantly reduced use of hospital services and costs. Included in the webinar will be exploration of the success factors, the challenges faced, and the future expansion of this model initiative. Space is limited - register today to attend this event.

The toolkit also includes public health examples and templates to address needs identified during PHF’s work in the field with public health and healthcare practitioners.
