Overview

​This Public Health Foundation (PHF) discussion paper, "Using a Population Health Driver Diagram to Support Health Care and Public Health Collaboration," was published by the Health and Medicine Division (previously known as the Institute of Medicine) of the National Academies of Sciences, Engineering, and Medicine in February, 2015. 

 

A population health driver diagram identifies primary and secondary drivers of an identified community health objective, and serves as a framework for determining and aligning actions that can be taken across disciplines for achieving the objective. Grounded in the belief that health care and public health are more effective when they combine their efforts to address a health issue than when they work separately, population health driver diagrams can be used to tackle challenges at the crossroads of these two sectors.
